To properly prepare for your upcoming live scan fingerprinting appointment, you will need the following documents:
- BCIA 8016 “request for live scan fingerprints” service form
- Social security number
- Acceptable valid photo ID

The following are important fields that are to be completed:
- ORI Code – The agency’s “applicant record identifier” assigned by the DOJ
- Type of application & job title
- Agency address that will receive your applicant background information
- Mail code the agency’s 5-digit DOJ assigned code
- Contact information, name of the agency contact person
- Telephone, agency’s contact person telephone number
- Your information, name and description
- Your social security number
- Your driver’s license number
- Referring organization’s billing number ( if they are paying for your background check fees )
- Your phone number
- DOJ and / or FBI level of service selection
You must list your social security number on your BCII 8016 Request for Live Scan Service form. This is a DOJ and FBI mandatory requirement. If you do not provide your social security number you will not receive live scan fingerprint service. You must also present a valid photo ID to the live scan fingerprinting technician.
The following are examples of acceptable forms of photo identification:
